Anticoagulants

Enquire as to why the patient is taking anticoagulants (may also need antibiotic cover for valvular problems).

 

Perform an INR. If result is <3.0 and the surgery is of a very minor nature, then adjustment of the Warfarin dosage is not required.

 

If the INR >3.0 or the surgery is more major then stop Warfarin and commence a full Heparinisation regime. Re-institute Warfarin post-operatively. Consult specialist patient is under and use Tranexamic acid mouthwashes.
